Admission - Application

The Open Enrollment period is from January 30, 2012 through March 14, 2012. Applications will not be accepted or considered if received prior to January 30, 2012. Applications received during the open enrollment period will be weighted equally.

If you are interested in submitting an application for your child to attend Discovery Charter School for the 2012-13 school year, please complete the following steps:

**Please note that the California state school entry date for Kindergarteners has been changed. Effective as of the 2012-13 school year, children must be five years of age on or before November 1, 2012 to eligible for Kindergarten.**

STEP 1

Download the Application Packet, fill in electronically as much as possible, and then print it out. All of the Application documents are in PDF format. If your computer does not have a PDF viewer, go to www.Adobe.com for a free PDF document viewer.

Application Packet Documents:

1. Application Checklist/Instructions (Informational)
2. Application Form (Required)
3. Responsible Party Information Form (Required) - attach additional sheets as needed
4. Signed Parent Agreement (Required)
5. Signed Fingerprint/Background Check Release Form (Required)
6. Application Health Requirements (Informational)

Additional Application Requirements:

1. Attendance at a School Information Night meeting (Required, except for returning families.)
2. Attendance at a School Open House (Required, except for returning families.)
3. 2 Proofs of Residency (Only required for Moreland School District residents)
4. Birth Certificate for incoming Kindergarteners, students from private schools, or students coming from outside the U.S.

Additional Information:

All kindergarteners and their parents are expected to attend a kindergarten readiness meeting and a parent meeting to be held prior to the start of school. If you are a kindergarten applicant, you will receive additional information by U. S. mail regarding these meetings.

STEP 2

Carefully review the application checklist included on page one of the Application Packet for a complete list of required application forms and application information. Carefully fill in the requested information on all forms as indicated.
